Staff Pharmacist

LTC Pharmacy Services, LLC is a new long-term care pharmacy services provider in Milwaukee, Wisconsin, dedicated to providing quality patient care to our residents in long-term care settings. We are looking for a motivated, dynamic pharmacist to join our team!

Minimum qualifications:

•  BS or PharmD degree from accredited school of pharmacy

•  IV clinical experience preferred

•  Long term care experience preferred

•  Current pharmacist license in state of Wisconsin

•  Must be available to work the following schedule: Monday-Friday 8am-8pm and Saturday 9am-5pm. Scheduled hours subject to change according to pharmacy needs.

•  Strong analytical and critical thinking skills

•  Excellent verbal and written communication

•  Excellent attention to detail and organizational skills

•  Ability to multi-task and contribute to workflow

•  Excel in a fast-paced environment-manage deadlines and prioritize tasks

Essential functions:

•  Provide pharmaceutical care that includes therapeutic drug monitoring, pharmacokinetic evaluations, and documentation of patient medical records

•  Respond to pharmaceutical inquiries from patients, medical personnel, and pharmacy staff in a professional and friendly manner

•  Evaluate medication orders for appropriate drug choice, dosage, and potential interactions

•  Ensure accuracy of medication orders entered into computer system

•  Ensure medications are accurately and efficiently dispensed

•  Oversee activities of other pharmacy personnel

•  Handle incoming physician call, log data into the computer system, and perform drug utilization review

•  Perform other tasks as assigned

•  Conduct job responsibilities in accordance with the standards set out in the company’s code of conduct, applicable federal and state laws, and applicable professional standards
