Job Description:
• Perform client assessments, review physician orders, and create care plans
• Prepare, record, and maintain clinical and progress notes in the client care record
• Serve as case manager, responsible for coordinating care for assigned caseloads
• Observe and report on signs or symptoms indicative of changes in condition to the physician and document such reports and observations in the client record and update the care plan
• Educate clients and family members on resources, medications, self-care techniques, and/or medical conditions per physician orders
• Identify discharge-planning needs as a part of the client care plan implementing them in accordance with physician orders
• Play a key role in clients avoiding unnecessary visits to the ER and or hospitalizations
• Perform case supervisory visits on a periodic basis throughout the year
• Act as a role model for new caregivers, accompanying them on home visits and providing mentorship
• Meet required deadlines for documentation of certification, re-certification, supervision reports, care plan updates, addressing progress towards goals, and physician orders as needed
• Build personal rapport and establish trust with clients, family members, and caregivers
Required Qualifications:
• Registered nurse with current licensure to practice professional nursing in New York State
• Minimum 1-3 years of nursing experience (home health, home care, and/or hospice preferred)
• Must possess and maintain current CPR certification
• Knowledge, ability, and judgment to perform all nursing procedures under the direction of a physician as outlined in the scope of practice in the New York State Nurse Practice Act
• Meet the requirements for employees providing direct client care
• Valid driver’s license and use of an insured automobile for work or access to adequate transportation
