Job Description

Looking for LCSW or LCSW-A (with experience and close to full licensure) to work in a fast-paced, Federally Qualified Health Care Center (FQHC) in an integrated and team-based care environment. The right candidate would be passionate, dedicated and have a genuine desire to serve and care for a diverse patient population. As an FQHC, we serve a variety of patients on a sliding fee scale and take Medicaid, Medicare and commercial insurances.

We are looking for an outpatient therapist to work as part of our behavioral health team. Therapist will work closely with care managers and patient navigator and physical health care providers (pediatrics, women’s health and adult care). Therapist will complete assessments and provide brief and long-term therapy to patients ranging in age from childhood-adult (primarily will see adults; some work with children, preferred but not required). Therapist will work closely with physical health providers to meet patient’s whole health needs (for example, may include medication adherence goals to manage a patient’s Diabetes as well as traditional CBT/DBT interventions to address Bipolar Disorder, ADHD, Major Depression, etc).

Variety of opportunities for team-based care initiatives working with special populations including: patients diagnosed with Intellectual and Developmental Disabilities, members of the LGBTQIA community, undocumented individuals and families, and patients needing Medication Assisted Treatment (MAT) among others. Experience with substance use disorder preferred and certification as LCAS or LCAS-A would be ideal. Many of our patients are Spanish-speaking and being bilingual would be helpful but is not required. Significant opportunities for continuing education and innovative ideas/projects are encouraged to meet the needs of our patient population.
