The Nurse Care Coordinator reports to the Nursing Team Leader. The NCC coordinates medical care for persons discharged to Thresholds from nursing homes as part of the Williams Consent Decree. The NCC will work with staff on community support teams and members to ensure that health needs are assessed, coordinated and documented.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Monitor and facilitate primary health care and psychiatry services for Thresholds members.
- Establish and oversee a process for communicating pertinent patient information and status reports to designated Thresholds and community healthcare providers, including psychiatrists.
- Conduct nursing activities such as assessments, medication administration, patient and significant other education.
- Work with the IMD Transition Coordinator to provide medical assessment/screenings prior to a person being transitioned to Thresholds.
- Consulting with IHC and other healthcare providers regarding patient management and follow‐up.
- Participating in project training and meetings as necessary.
- Documenting patient care and other activities per Thresholds policies.
- Collaborate with Thresholds community support teams that will work with Williams’ members.
- Other duties as assigned.
MARGINAL DUTIES & RESPONSIBILITIES
- May be required to work flexible schedules and be available to work on weekends or evenings as required.
Requirements
EDUCATION
- Bachelor's degree from an accredited school of Nursing. Master's preferred
EXPERIENCE
- Experience working with persons diagnosed with mental illness preferred.
- Ability to synthesize and summarize information and make judgments regarding patient care.
- Must communicate effectively both verbally and in writing in order to provide information to patients and healthcare personnel.
- Demonstrated ability to function independently.
- Computer skills required; knowledge of database functions and maintenance necessary.Daily access to a personal vehicle is required.
- Must obtain and maintain $100,000/$300,000/$100,000 liability insurance coverage. Subject to validation every 6 months.
SKILLS/CERTIFICATIONS
- Licensed Registered Nurse required.Current and valid driver's license required.
