POSITION DESCRIPTION

The Herbert Wertheim College of Medicine is currently seeking a Physician Assistant to join our team of professionals.

Duties Include:

Collaborates with NHELP treatment inter-professional team in all phases of assessment, planning and treatment. Performs a complete psychosocial historical review and physical assessment. Obtains and record medical and social histories for patients of all ages which have implications for treatment of the disease processes (including social determinants of health) for which the patients are being seen.
Performs and records a complete or problem focused physical examination for patients of all ages. Orders and/or performs, evaluates, interprets, and integrates findings of diagnostic tests and procedures.
Manages emergent/urgent/acute health problems and documents care, under the direction and protocols of the supervisory physician. Provides patient/family education and counseling with full consideration of patient safety needs under the direction of the supervisory physician.
Discusses patient care and social/educational/legal needs with physician and/or other health or social service professionals when appropriate.
Refers and navigates patients to physician or to specialized healthcare or psychosocial resources for consultation/treatment/services as appropriate.
Prescribes medication as permitted by the Florida Board of Medicine under the direction of the supervising physician.
Performs standing orders or other orders delineated by the supervisory physician. Assists in behavioral health events, management, and procedures. Establishes a health status database that is pertinent to visit.
Maintains patients' medical records according to established standards and documents in a systematic, accurate and succinct format. Reviews records with supervisory physician on a routine basis.
Maintains an environment of safety for patients, Participate in continuing education as required by licensing body and medical staff bylaws.
Evaluates and makes recommendations for a household-centered socio-medical shared care plan with full consideration of patient safety needs under the direction of of the supervisory physician.
Coordinates the implementation of household-centered socio-medical shared care plans under the direction of a supervisory physician.
Coordinates with NHELP outreach team to incorporate outreach interventions into care plan. Communicates with non-NHELP providers to coordinate care, as appropriate. Coordinates with behavioral health team to integrated behavioral health intro assessments and care plans as appropriate.
Performs other related duties as assigned or as directed. The omission of specific duties not preclude the supervisor from assigning duties that are logically related to the position.

MINIMUM QUALIFICATIONS

Graduate of a Physician Assistant Program accredited by the Accreditation Review Commission on Education for the PA and one (1) year of related experience.
